When should you remove wisdom teeth in Shrewsbury, PA?

Whenever wisdom teeth have no room to erupt and enter the mouth as fully functional teeth we will recommend removal. We advise waiting long enough to determine if the teeth will come in normally prior to making the decision to remove wisdom teeth. Wisdom teeth will erupt between the ages of 16 and 19 usually. We recommend a panoramic x-ray image be made to assess the development of wisdom teeth to determine if removal is the right decision for you.

BONE GRAFTING WITH TOOTH EXTRACTION

In this new era of dental implant tooth replacements, dentists have to consider what the patient wants to do after an extraction is performed. Often a dental implant or a bridge will be considerations for filling the gap where a tooth was lost. The results of a tooth extraction is alveolar bone loss- the bone shrinks away! Research has shown that up to 50% of the supporting bone necessary for anchoring an implant will dissolve and atrophy in the first year alone. If a patient is planning on an implant at any time during the future, alveolar ridge preservation or ridge preservation grafting should be planned along with your extraction. How is dental bone grafting done? Between .5-1.0 cc of bone graft material is placed in the extraction site and is usually covered by a surgical semi-permeable membrane that was originally Gore-tex and is now either a collagen or a Teflon perforated membrane that permits bone growth to occur while eliminating the tendency for the body to allow gum tissue to fill the socket and result in lost bone. Understanding how bone healing works allows us to perform guided bone regeneration that preserves and can augment sites where we plan to place dental implants. The area is closed with sutures and allowed to rest for 3 weeks on average. A second appointment will be made for suture removal and you may have your implant placed as early as 6 to 8 weeks after your extractionsocket grafting procedure.

When patients have decided that keeping their original teeth is no longer possible, a denture or implant denture is a common choice these days. We prefer to coordinate the entire process of creating an immediate denture and removing the teeth at the same time for our patients. The process is different from just removing one tooth.

Multiple teeth being extracted at one time will require alveoloplasty (smoothing the underlying bone) or the gum tissue will not be smooth when the gums heal and the denture sore spot will become a huge problem if you could wear a denture at all. Dental bone grafting is often considered when extracting all the teeth to help preserve and rebuild the bone that will support a denture or an implant retained denture. The gums are sutured in the proper position for the next 2 -3 weeks and finally your immediate teeth placed.

Our patients have new “teeth in a day” but must wait for full healing prior to us moving forward to final restorations. Pain medications and antibiotics will often be prescribed and patients can be back to normal life in a few days. Final healing of the gums may take several months. We oversee the healing for the entire post-operative period. Transitioning out of your original teeth and into new teeth is a journey and we are here for you along the way to help you achieve the new smile you want and deserve.
